first visit you will normally receive an oral exam by one of our dentists.  
If you have X-Rays from your last dentist, please bring them with you 
(or have them forwarded to our office). 

If you have dental insurance, please bring your card with you to your 
first visit.  For your convenience, you may download several forms that 
are normally filled out at your initial visit.  Or, if you like, come to your 
first appointment about 15 minutes early to fill them out.
